About Your Visit

Prior to your visit:

Complete your health forms that will be sent to your email.

Your provider will:

Review your health forms and reason for seeking our care.
We will develop a shared plan of care.
Send medications if appropriate.

Branded medication requests:

We will send branded medications to your local pharmacy at your request.
Call your local pharmacy and verify your price and coverage for the branded medication.
Verify that your pharmacy has the branded medication in stock.
You will be required to pay for a visit with each branded medication prescription or prior authorization request.
90-day prescriptions will be prescribed if requested at the time of your visit.
